У меня есть абсолютно точный ответ на этот вопрос. Форма ГринКарты как у кредитной карточки, прямоугольная с закруглёнными углами.

No more forms(I mean on paper) . You have to fill entry electronically between Nov1 and Dec. 30 on www.dvlottery.state.gov

Your link is just some agency. I can do the same for $25. I guess Vlad's link won't work untill registration time
Anyhow, I think that all info is here:

and I think it's pretty cool, that they have the online submission... though it does have some negatives along with positive sides... ..hopefully # of stupid people who cannot use the computer is more than the # of stupid people who cannot [correctly] fill out paper application.
